After viewing you should be able to understand the error chain that can lead to an accident or incident, the human factors that influence pilot decision making, and effective ways to break the error chain. Excellent tool to be view as a group and discussed or individually. Five scenarios are contained. 1) Pax Seat Driver. A chartered Learjet is carrying a new customer to a non-towered airport. Weather issues and discussion may preclude landing of the destination airport, while controversy surrounds the crew. 2) Diverging Opinions. Airline service to a new market, inoperative components, time pressures, weather, crew coordination, and arrival delays complicate the otherwise routine flight. 3) Cockpit Clash. A corporate turboprop with a new crew pairing are preparing to take a nervous customer to the companies retreat. Cerw incompatabilities, and a warning light in the cockpit complicate the flight. 4) Shifting Responsibilities. On the maintenance floor of a commuter airline the shift change has just taken place. Numerous inspections, line operations pressures, and maintenance duties overtax the reduced staff. 5) Bumpy and Grumpy. On a regional airline flight, the crew has numerous personal problems including time constraints, new employees, and FAA inspector and maintenance issues.
